The challenges of group psychotherapy in an adult psychiatric residential treatment facility are complex and multifaceted. A significant determining factor is the cohesion and consensus among the pivotal participants within the group, from the leader to the supporters, and various role-players.
Strong cohesion within the group’s core can determine whether the sessions are favorable or negative. The facilitator bears the responsibility of initially navigating these primary relationships while remaining cognizant of the equal distribution of power and relational dynamics. It is crucial not to cater to a single dominant personality, instead taking a guiding but structured approach to the therapeutic process. By leveraging these relationships constructively, the participants can contribute to a positive therapeutic outcome.
Challenging behavior that is consistent with instigating can attempt to manipulate or monopolize a group, disrupting the harmonious cohesion. This is similar to how a nail can deflate a perfectly inflated tire, which represents the collective circle of inclusive group cohesion. Just one disruptive element can drain the group of hope and optimism.
